SIR:- In compliance with the requirements of an Act of the General Assembly of the Commonwealth of Pennsylvania, entitled "An Act to provide for registration and protection of names, titles or designations of associations, societies, orders, foundations, federations, organizations and corporations of the first class approved the 16th day of May, A.D. 1923, the undersigned Theta Chi of Penn State, a proposed corporation of first class having heretofore adopted or used or desiring to adopt or use a name, title or designation for the purposes provided in said act, and desiring to register the same in the Office of the Commonwealth of Pennsylvania do hereby certify:
That the name, title or designation of the association, society, order, foundation, federation, organization, corporation of the first class or proposed corporation of the first class so registering is THETA CHI OF PENN STATE
Its address location or place of business is 803 Bailey Building, 1218 Chestnut Street, Philadelphia, PA
The length of time, if any, during which he said name, title or designation has been used y applicant is five (5) years
The nature of the applicant's business or activities is to maintain a club for social enjoyment and to promote fellowship kong those embers of Omega Chapter of Theta Chi fraternity who shall of been graduated or otherwise severed their connection with Penn State College

Personally appeared before me this seventh day of January A.D. 1295: John A. Irwin who duly sworn , according to law, deposes and says, that the statement contained in the foregoing interment are true; that the proposed corporation of 1st class so filling such name, title or designation has the right to use the same and no other identical form or in any such near resemblance hereto as may be calculated or decieve.
